If your child has been prescribed medicine that needs to be taken in school time, please fill in and return the Medication Form to the office in person or email office@haydonleighschool.co.uk

Consent to walk home form

If your child is in Year 5 or 6 and you would like them to walk home alone at the end of the day, you must complete this form and return it to office@haydonleighschool.co.uk

Mobile phone permission form

If your child travels to school on their own and brings a mobile phone, please complete the form and return it to office@haydonleighschool.co.uk

Absence Request form

Please complete the Absence Request Form below for any requests for absence during term time, that are exceptional circumstances, and either email it to office@haydonleighschool.co.uk or hand it into the school office. All requests must be made at least 2 weeks before the start of the absence.
