Walking Bus form Term 2 2023/2024

If you would like your child to join the Walking bus, please complete the form and return it to office@haydonleighschool.co.uk.

Medication form

If your child has been prescribed medicine that needs to be taken in school time, please fill in and return the Medication Form to Mrs Pickering via office@haydonleighschool.co.uk

Consent to walk home form

If your child is in Year 5 or 6 and you would like them to walk home alone at the end of the day, you must complete this form and return it to office@haydonleighschool.co.uk

Mobile phone permission form

If your child travels to school on their own and brings a mobile phone, please complete the form and return it to office@haydonleighschool.co.uk
